Why good practices fail.
Every software project that failed had a plan. It had a specification. It had coding standards, a testing strategy, a documentation process, and a review protocol.
And then the deadline moved. And the team cut corners. First the tests. Then the documentation. Then the spec reviews. Then the architecture checks. Not because anyone decided to, but because under pressure, humans optimize for speed by dropping the things that feel optional until they aren't.
This is the real reason most software projects fail. Not bad developers. Not wrong technology. Not poor management. The practices that prevent failure are the first things that get sacrificed when failure becomes most likely.
We've known this for decades. And we've spent decades trying to fix it with process: Agile, Scrum, SAFe, Definition of Done, pull request templates, QA gates. Layer after layer of process designed to keep humans honest.
It didn't work. Not because the practices were wrong, but because they depended on discipline under pressure. And humans, no matter how talented, don't maintain discipline when the sprint is late and the client is calling.

The conversation is stuck on speed: AI writes code faster. That's true, but it's the least interesting part.

Most software houses in 2026 still sell you a team: five developers, a PM, a QA engineer, billed monthly. They report velocity in story points. They explain delays with "complexity" and "technical debt." The billing model rewards time spent, not problems solved.
We think this model is ending.
Not because developers are unnecessary. They're more necessary than ever.
